Documents should be submitted with the application -:

Dully filled, printed and signed application
Payment slip (application fee)
Certified copies (from a Justice of Peace, Notary Public, Government Officer at Executive Level) of;
- Your Degree Certificates and other qualification certificates
- Your Birth Certificate
- Your NIC/Driving License/Passport

Documents should be handed over to the FGS faculty office/sent by registered post.
